Important Reminders in the Checking of School Forms

⚫ Only SF-1, SF-4, SF-5, SF-6 and SF-10 are subject for checking

⚫ SF-2 and SF-9 are just secondary report and need not be checked unless for validation purposes

⚫ SF-2 will be used only as reference in case of inconsistencies in SF-1, SF-4 and SF-5

⚫SF-9 can only be checked if SF-5 and SF-10 did not match

⚫SF-3 & SF-8 are not supposed to be reviewed or checked by SCC and DCC

PSA/NSO Birth Certificate is preferred but not required or mandatory.

Other equivalent document can be used as reference to establish identity of Learner.

Mandatory DCC membership of SEPS for Planning and Research and the DPO III

LIS/ICT coordinator must be assigned as Vice-Chair of SCC

2✓ LEVELS
Only 2 levels of checking;

School Level (all classes) to be conducted by the School Checking Committee (SCC)

Division Random Checking (all schools but randomly selected classes only) by the Division Checking Committee (DCC). 

Checking Committees
Focus on the accuracy and consistency of learner information, instead of cosmetic aspects of the school forms (SF).

LIS-generated school forms with pre-loaded learner information are not subject to editing outside the LIS. Any corrections should strictly be done in the system.

When printing, use black ink on A4 (8.27" x 11.69") or long/folio (8.5" x 13") white bond paper

Class Advisers
You are not required to prepare additional local forms for the ranking of honors

The SF-5 upon validation against the SF-10, can be used to evaluate and determine the learners who are candidates for honors and other citations.

The traditional practice of ranking has not been applicable since SY 2016-2017, so, schools are not required to prepare additional local forms for the ranking of honors.

You are not required to prepare and submit obsolete school forms that were replaced by modified school forms through DepEd Order No. 4, s. 2014.
